2. Download the Form(s)

Diagnostic Systems of Georgia wants your visit to go smoothly and we respect YOUR time.

You can click the link to the right for the forms that will be necessary to complete your visit with us. Feel free to go ahead and print your forms for registration, health screening and the forms specific for the exam(s) you will be having. Should you have any questions about these forms, please contact the DSGA center nearest you and one of our customer servivce specialists will be glad to assist you.

3. Choose a Payment Option

Diagnostic Systems of Georgia is happy to offer you many options for payment. Prior to your exam, one of our trained financial counselors will contact you with information regarding your out of pocket responsibility and will go over these options…

  • You may pay your ESTIMATED balance in full on the day of your exam. We accept cash, check, Visa/MasterCard/Discover, and debt cards.

  • You may opt to use our 12 month INTEREST FREE medical financial assistance program through Care Credit. There is a short application that can be done online or at your appointment. Once you are approved, you may use this medical payment account at other providers that accept Care Credit.

  • You may chose to pay 50% of your balance the day of your exam with the remaing 50% due in 30 days.

Our Financial Counselors are there to assist you and will work with you to find the right payment plan for your needs. Please call your nearest DSGA location to learn more

4. Schedule Your Exam

  • Make an Appointment

    You are welcome to call your local DSGA center to schedule your exam. One of our customer service representatives will be happy to help. Please choose the center nearest you from our locations listed at the bottom of this page for the contact information.

    Email your request to us... it's simple and quick! Choose the nearest DSGA center from the links below and submit your email directly to that location... one of our customer service representatives will respond to your request within 2 business days.

  • Already Have an Appointment?

    That's great! You're a step ahead. Please use the site as a resourse to find out more about your exam, any preparation you may need for the exam, directions to your DSGA center, and payment options. You are also welcome to download the forms you will need to complete prior to your exam. We encourage you to print these and have them ready when you come in ... and as always, please call us with any questions or concerns you have regarding any of the topics listed above. We look forward to seeing you!

5. After the Exam…

Once your exam is completed, one of our Subspecialty Trained Radiologists (this means they are specifically trained to look at the area of your body that they exam was performed on) will interpret your exam and a written report will be sent to your physician.  Your physician's office will contact you regarding the results or they will go over the results with you at your next appointment.  We are not at liberty to discuss the results of your exam.. we ask for your understanding.

DSGA will bill your insurance on your behalf.  This process generally takes 35-40 days.  You will receive a document called an Explanation of Benefits (EOB) from your insurance company once they recieve our bill.  This will reflect the amount charged, the amount 'allowed', and what portion is your patient responsibility.  You will have already paid all or most of this amount so this will allow you to reconcile any remaining amount you may owe.  We will send you a statement of your account that reflects the same information as your EOB which will serve as your bill for any remaining balance not previously paid.
